Payment and Insurance
Payment Methods
Dr. Gates accepts cash, check, Visa, MasterCard, Amex, and Discover.
Payment is due at the beginning of each appointment.
Out-of-Network Provider
Dr. Gates is an out-of-network psychiatrist. This offers the opportunity for those who want complete confidentiality to pay out of pocket. Patients may also choose to submit invoices for services (superbills) to their insurance carrier to receive reimbursement; patients are encouraged to contact their individual insurance plan to learn about their out-of-network benefits.

About Superbills
When requested, a superbill is provided for the services that will include: cost of service, payment received, provider NPI (National Provider Identifier number), Business EIN (Tax ID#), Date of service, "CPT" Billing Codes (describing Type of Services rendered) and "ICD" Diagnosis Codes.
It is the patient's responsibility to submit this invoice to your insurance company for out-of-network reimbursement, which often reimburses 20-90% of total cost. Reimbursement typically takes 30-90 days.
